No negative ion signals corresponding to genistein or glycitein aglycones or any isoflavone glucuronide were observed. Similar weak signals for sulfate conjugates were seen in the blood of two other volunteers not shown ; . Weak signals consistent with genistein sulfate and daidzein sulfate were observed in a subsequent experiment using LC-MS with selected ion monitoring of the M H ; ion not shown ; . The corresponding control blood sample showed none of these responses Fig. 5, left ; . Table 5 shows the serum concentrations in two volunteers of isoflavones present as the aglycones, the sulfates, the glucuronides, and the totals. These values were obtained by using either no enzyme, a partially purified sulfatase, a recombinant glucuronidase, or a mixture of sulfatase and glucuronidase, respectively, for deconjugation. The method validation and performance specifications were previously reported in a study of isoflavones in rat blood Holder et al., 1999 ; . Their study also showed that the recombinant glucuronidase catalyzed quantitative hydrolysis of authentic glucuronide standards under the conditions cited above. In addition, the purified sulfatase preparation was devoid of glucuronidase activity. The same volunteers consumed the soy supplement on another date, at which time the blood metabolite profiles and determined values were comparable not shown ; . In accord with the greater daidzein than genistein content in the soy supplement 6-fold ; , the total daidzein in blood exceeded.

More research is still needed to confirm these possible benefits, and women should not rely on soy for protection from osteoporosis. Note: supplements containing only individual isoflavones do not appear to provide any benefits. ; Alcohol. Alcohol has different effects on bones depending on how much is consumed. One 2000 study found that women older than 65 who drank one to two drinks 1 to 2 alcohol weekly had higher bone density than non-drinkers. Alcohol in moderate amounts may reduce parathyroid hormone and increase estrogen levels. Excessive drinking, however, has been associated with brittle bones. Cola, Coffee, Tea and Caffeine. A 2002 study suggested that drinking tea regularly may help protect bones. Nevertheless, there has been some concern that caffeine consumption, particularly from coffee, may increase calcium levels in urine and reduce levels in the body. In one trial, consumption of lots of coffee, nine or more cups per day, was associated with an increased risk of hip fractures in women, but not in men. However, not all studies support a risk. Some evidence, in fact, suggests that caffeine may pose a danger for bone loss only in elderly thin women--but not in those who have normal or high weight. Drinking carbonated beverages, particularly cola, may increase the risk for bone fractures in people with low bone density.

Of the tips of fingers and toes, in which the angle at the nailbed is lost and the tips expanded, is one of the best known nail abnormalities associated with disease. Hippocrates was the first person to describe this abnormality, but it took until the 19th century before the association of clubbing with periostosis of the distal ends of long bones, synovitis of large joints and occasionally a thick sebaceous skin was established. The Frenchman, Pierre Marie, recognized this syndrome as being clearly distinctive from acromegaly and named the entity hypertrophic osteoarthropathy HOA ; [1, 2]. Whether clubbing is a separate disease or an incomplete form of HOA remains unclear. However, as HOA often starts with isolated clubbing, and because both conditions can be secondary to the same underlying disease, certain authors have suggested that clubbing and HOA are in fact manifestations of the same disorder. Moreover, the radiological features and the periungual tissue histopathology are indistinguishable [3-5]. Secondary clubbing can be seen in a number of cardiopulmonary and gastrointestinal disorders, as well as in some infectious diseases. Chronic viral infections, however, are not yet recognized as an underlying cause. Here, we report two patients infected with HFV who presented with clubbing.

"If Palm Beach Gardens Medical Center has bugs in operating rooms, ants crawling around ICUs, rat bait stations outside the hospital and flies breeding in the mops, the public has a right to know, " declared Cal Warriner of Searcy Denney Scarola Barnhart & Shipley. "Sometimes a request for a gag order backfires. The effort to quash negative publicity arising from the suits' allegations provided a platform for the lead plaintiff counsel ; to describe alleged conditions at the hospital that would make anyone gag in disgust, " opined the Daily Business Review, which described the hospital's failed effort as "a public relations disaster." To better understand the consequences of long-term soy isofkavone exposure on the molecular signaling events associated with breast cancer, we developed an in vitro cell line model. As part of this model, sublines of the human MCF7 and MCF10AT breast cancer cell lines were generated by chronic exposure to low concentrations of genistein 10 nmol L ; . During the characterization of these sublines designated MCF7G and MCF10ATG ; , stable changes in cell phenotype conferring altered growth properties and morphologic characteristics were observed. These changes were associated with increased sensitivity to treatment with paclitaxel, a drug commonly used for breast cancer chemotherapy. Specifically, paclitaxel-induced growth inhibition, apoptosis, and G2 M arrest in MCF7G and MCF10ATG cells were twofold greater than those observed in the parental cell lines. Moreover, the changes detected appeared to be stable subline characteristics, as demonstrated by their persistence after removal of genistein from the media. Genistein potentiated the paclitaxel-induced G2 M arrest. In cells that were treated with genistein and paclitaxel, genistein pretreatment potentiated the inhibitory effects on the same parameters in both the subline and parental cells, exceeding the responses to treatment with either agent alone. If the sequence was reversed and genistein treatment followed that of paclitaxel, the G2 M fraction became diminished and very little apoptosis could be detected. One explanation for these findings is that when genistein precedes paclitaxel it leads to S phase entry and paclitaxel-induced arrest. When genistein follows paclitaxel, it promotes G2 M escape and reduces apoptosis. We are also evaluating the effects of genistein on the reduction of paclitaxel resistance. Observational studies and small trials of short duration suggest that the isoflavone phytoestrogen genistein reduces bone loss, but the evidence is not definitive. This Italian randomized, double-blind, placebo-controlled trial assessed the effects of genistein on bone metabolism in osteopenic postmenopausal women. A total of 389 postmenopausal women with a bone mineral density BMD ; less than 0.795 g cm2 at the femoral neck and no significant comorbid conditions were included. After a 4-week stabilization period during which participants received a low-soy, reduced-fat diet, participants were randomly assigned to receive placebo n 191 ; or 54mg of genistein n 198 ; daily for 24 months. Both the genistein and placebo tablets contained calcium and vitamin D. The primary outcome was BMD at the anteroposterior lumbar spine and femoral neck at 24 months. Secondary outcomes were serum levels of bone-specific alkaline phosphatase and insulin-like growth factor I, urinary excretion of pyridinoline and deoxypyridinoline, and endometrial thickness. Data on adverse events were also collected. At 24 months, BMD had increased in genistein recipients and decreased in placebo recipients at the anteroposterior lumbar spine change, 0.049 g cm2 [95% CI, 0.035 to 0.059] vs. 0.053 g cm2 [CI, 0.058 to 0.035]; difference, 0.10 g cm2 [CI, 0.08 to 0.12]; P 0.001 ; and the femoral neck change, 0.035 g cm2 [CI, 0.025 to 0.042] vs. 0.037 g cm2 [CI, 0.044 to 0.027]; difference, 0.062 g cm2 [CI, 0.049 to 0.073]; P 0.001 ; . Genistein statistically significantly decreased urinary excretion of pyridinoline and deoxypyridinoline, increased levels of bone-specific alkaline phosphatase and insulin-like growth factor I, and did not change endometrial thickness compared with placebo. More genistein recipients than placebo recipients experienced gastrointestinal side effects 19% vs. 8%; P 0.002 ; and discontinued the study.
